Methods of Payment:

Services can be paid for using U.S. currency, traveler's checks, Visa, MasterCard, Discover, American Express, money orders, and personal checks.

 

Refunds:

Refunds on training sessions and group classes are given only under extenuating circumstances such as illness, injury or relocation away from the area. Arrangements and exceptions for refunds are established on a case-by-case basis.

 

 

Insurance:

Chiropractic care maybe covered by your insurance plan. Please call to confirm coverage and eligibility in advance.

Cancellations:

All cancellations require 24-hours notice by phone. In the event that you do not give 24-hours notice, you will be charged the full amount of the scheduled appointment on your credit card that is on file.

You will receive a 12-hour notice by phone should they need to cancel your scheduled session.

 

Promptness:

It is important to arrive on time for a scheduled appointment. In the event that you are late, Optimal Fitness will be obligated only for the remaining period of time the session was scheduled for and you will be charged the full amount for the session.

 

Referrals:

The comprehensive nature of your training program may necessitate that you to work with additional practitioners (nutritionist, physical therapist, applied kinesiology, chiropractor, massage therapist, acupuncturist, etc.) either before or in conjunction with the training program in order to effectively reach the goals you have established. If it is determined that outside professional help will be needed, you will be encouraged to see the recommended practitioner's. This will facilitate the establishment of your goals and allow for a results-oriented training program.
